  • Employee Needs Assessment: Sample Questionnaire: Published by the British Heart Foundation. A questionnaire for collecting information about current physical activity behaviour, health needs, and physical activity interests and profiles. 

Assessing Your Work Environment

  • Organizational Health Audit  (Trifit): This audit assesses your organization’s corporate culture; policies, procedures and work processes; programs and services; and physical environment. The audit helps you recognize how your workplace already supports employee health and identifies opportunities for improvement.

  • Physical Activity Audit Template : Use this basic survey to measure current physical activity in the workplace and the facilities and resources in and around the workplace.

  • Workplace Assessment: Sample Questionnaire  (British Heart Foundation): This survey helps you put together a workplace environmental assessment by analyzing your organization’s structure and current health promotion practices, facilities and related resources.

  • Workplace Physical Activity Framework Audit Tool: A resource from the Alberta Centre for Active Living. This tool can be used as a baseline measuring tool and for continual evaluation.

  • Canada Awards for Excellence: Awards presented annually by the National Quality Institute (NQI) to private, public and not-for-profit organizations that have displayed outstanding performance in promoting a healthy workplace.
